Sequel has built and maintained relationships with community partners and agencies, such as state Departments of Health, Departments of Juvenile Justice, Departments of Human Services, and more that all work together to place the right individual at the right place at the right time.

We are continuously looking to enhance our partnerships and foster new relationships with the mission to help those in need of behavioral treatment to reach their best potential and provide them with the tools to lead productive lives.

Make a Referral to the Sequel Network

Residential Treatment Facilities

Referrals may be directed to each program’s Admissions Department via phone, fax, mail, or email. In order to expedite your referral, we request the following information be submitted for review:

    Background information
  • null
    Past placement history and reports
  • null
    Psychiatric/psychological evaluations (if applicable)
  • null
    Court documents
  • null
    Other supporting documentation

After reviewing the information, we may request a face-to-face or telephone interview as well. In most cases, a referral decision will be made within 48 hours of initial contact with Sequel.

Once the decision is made, an admissions staff member will notify you by telephone and will send written verification by mail. You will receive a set of consent forms, a questionnaire and visitation policy that must be reviewed, completed, and signed by the parent/guardian and/or you, and returned to the program Admissions Department prior to admittance. A projected admittance date will then be discussed.

Community-Based Services

Referrals for Community-Based Services in the Sequel Network can be submitted to the local program nearest you. Once you contact the program, someone will be in touch with you to learn more about your needs and how we can help.

Autism & Education

Referrals for Autism & Education programs in the Sequel Network can be submitted to the local program directly. Once you’ve contacted the program, someone will be in touch with you to learn more about your specific needs and discuss how we can be of service to you and your family.
